Gainesville, Georgia: Hyperandrogenemia due to ingestion of Butea superba

Androgen or testosterone is an important masculine hormone. It helps construct the male appearance. Hyperandrogenemia is majorly described in of females with hirsutism, insulin resistance and polycystic ovarian syndrome. However, hyperandrogenemia in males is not frequently mentioned. In this short article, the authors report a case of hyperandrogenemia due to ingestion of Butea superba, a herb found in South East Asia. This is an interesting case of hyperandrogenemia induced by an external source. The effect of phytoandrogens is also discussed.

The patient was a Thai single male, aged 35 years, without any underlying disease (his basic annual laboratory checkup showed normal results). On presentation, the chief complaint of this patient was a feeling of increased sexual drive. He gave the history of no use of narcotic and regular intake of vitamin and nutritional supplementation. Physical examination revealed no significant abnormality. Laboratory investigations were performed which showed increased dihydrotestosterone (1512 pg/mL, reference value 250–990 pg/mL). The results of other sexual hormone related investigations in this case included dehydroepiandrosterone sulfate 328 μg/dL, free testosterone 1.7% and sex hormone binding globulin 43.24 nmol/L [no data on follicle stimulating hormone (FSH) and luteinizing hormone (LH) levels]. The diagnosis of hyperandrogenemia in this case was therefore arrived at. Further investigation to find the source of androgen in this case was performed. With complete history taking, the clinical nutritionist could define an important problematic food component, B. superba, a local herb. The patient gave additional information that he had just taken this locally made capsule of this herb for a few weeks because he was suffering from hair loss. This patient was advised to stop ingestion of this herb, and follow-up after 1 week revealed that the patient had no feeling of increased sexual drive and dihydrotestosterone had decreased to normal level.

In general, androgen plays an important role in the sexual drive of males. Decreased androgen level is strongly related to reduced sexual activity and decreased sexual drive. This is a common problem in old males and in cases with erectile dysfunction. In males with the problem of sexual desire, androgen investigation is a useful test. In this case report, the patient also complained of increased sexual desire, which hmight have been due to exogenous hyperandrogenemeia.. Similar to hyperestrogenemia caused due to phytoestrogens, some herbal regimens might contain phytoandrogens that can lead to hyperandrogenemia. In this case, B. superba is the problematic external source of excessive androgen. This plant is considered to be a male potency herb. In animal models receiving this herb, stimulation of sexual organ has been reported. In case of human beings, there is only one previous trial using this herb for treating erectile dysfuntion. The effect of this herb is comparable to that of sildenafil. However, there has never been any report on this herb in healthy males. This is the first case report on hyperandrogenemia due to ingestion of B. superba. With the widespread usage of local herbs presently, the effects of herbs need to be considered. Indeed, a previous report also mentioned genotoxicity due to large dosage ingestion of B. superba.

He chose to travel to the controversial Dignitas clinic because he could not face the agony of the incurable disease

A British man has become the first dementia sufferer to die at a controversial suicide clinic.

The 83-year-old man ended his life at Dignitas in Switzerland because he could not face the agony of the progressive, incurable disease.

He also wanted to spare those closest to him from any burden and strain his illness might put on them.

The unnamed man, said to be from a wealthy professional background, was in the early stages of dementia.

He is believed to be the first to use the clinic’s services solely because of dementia.

And last night it was claimed his family, including his widow, backed his decision “100 per cent”.

The man took with him a report from a psychiatrist stating he was mentally competent to choose to kill himself.

And last night one campaigner told how the pensioner was “so grateful at the end.”

Retired GP Michael Irwin, 81, had arranged for him to see a psychiatrist to produce a report saying he was mentally competent.

He revealed that the man’s wife had made the travel arrangements for the trip to Zurich.

Mr Irwin, who did not travel with the couple, said yesterday: “His family were 100% behind him.

"I have spoken to his widow since and she felt that it was handled in a very dignified and proper manner.”

“She is extremely happy about how everything was arranged.”

He added: “I have been four times with people to Switzerland.

"Two were terminally ill, one was very disabled and one was in her mid 80s so I have seen how it is handled by the Swiss. It is a very dignified procedure.

“You have got to be a very determined person to be able or willing to make that kind of journey.

“He knew of how things would deteriorate and took what I think is a sensible decision… both for himself and his family.”

But news of the assisted suicide will cause outrage among right-to-life and healthcare campaigners.

Critics claim it carries the implication that those with dementia should consider killing themselves.

Experts point out that sufferers can live for years with the condition.

It is also likely to widen the debate over the circumstances in which assisted suicide should be permitted.

The vast majority people who have chosen to die at Dignitas are those with terminal illnesses such as cancer or severe physical disabilities.

Campaign group Care Not Killing described the development as “alarming”.

Mr Irwin – nicknamed Dr Death - claims to have helped at least 25 people to die at the clinic. In the past he has been interviewed by police, but never arrested.

Although legal in Switzerland, assisted suicide is a criminal offence in the UK and carries a maximum prison sentence of 14 years.

More than 800,000 people in Britan suffer from dementia – around one in ten of all those between 80 and 84.

Mr Irwin defended the pensioner’s right to take his life before his condition deteriorated.

He said: “It takes three or four months on average from the day you make an application until the actual day you die in Zurich.

"So when people have a chronic problem or a slow-developing condition such as motor neurone disease, dementia or are severely disabled you have a crucial time factor.

“It’s important to stress that with early dementia, you are still then mentally competent for quite some time to make a decision about going to Dignitas.

"It’s important that diagnosis is made at an early time to give an individual that choice.”

Lord Falconer, a former Lord Chancellor, launched a private member’s bill in the Lords earlier this month to make assisted dying legal for the terminally ill.

Novelist Sir Terry Pratchett, 65, who was diagnosed with Alzheimer’s in 2008, is also a supporter and has become a flagbearer in the campaign to change the law.

Mr Irwin, co-ordinator of the Society for Old Age Rational Suicide, says the legal right should be extended to elderly people suffering from medical conditions and those who are severely disabled or enduring unbearable suffering.

He added: “This topic of old-age rational suicide should now be openly discussed. Lord Falconer’s bill will be focusing only on the terminally ill.

"The other two categories, the severely disabled and the elderly with medical problems, should be equally well discussed nowadays, especially with an ageing population.”

The number of dementia victims in the UK is set to rise to more than a million by 2021 – and 1.7 million by 2050.

Mr Irwin argues that elderly sufferers may prefer thousands of pounds that would be spent on their care to go to their grandchildren.

He said: “The desire to ‘stop being a burden’ on one’s family, and to avoid squandering financial resources perhaps better spent on grandchildren’s further education, could become the final altruistic gesture, especially when combined with a wish to stop prolonging a life that is both futile and very unpleasant.”

He claimed: “Part of what makes a patient’s suffering intolerable could be the realisation that it is ruining other people’s lives.

"Then, a doctor assisted suicide could be a rational moral act.”

But critics fear that if euthanasia was legalised there would be pressure to widen the category of people to be included.

A spokesman for Care Not Killing said: “It’s hugely alarming and shows the real agenda of those seeking a change in the law.

"What they are looking for is assisted suicide or euthanasia almost on demand.

“We’ve been warning about an incremental approach, as once you change the law you get more and more cases like this, which is why we are so worried.

“We know that people who are vulnerable, disabled and terminally ill will be most under pressure.”

More than 200 Brits have died at Dignitas since it first opened in 1998.

Broadcaster Melvyn Bragg has previously said he plans to kill himself if he begins to suffer from dementia.

The arts presenter, 73, whose mother had Alzheimer’s disease until her death last year aged 95, said: “Legal or illegal, I will do it.”

He added: “We can’t keep sending people to Switzerland. We should say, given certain conditions, it’s fine.”

£5k and all over in 30 minutes

The price of a suicide at Dignitas is believed to be around £5,000.

But the full service, including funerals, medical costs and official fees, can be as high as £7,000.

Clients must register as a member and send copies of their medical records with a letter explaining why things have become intolerable.

A doctor then assesses them. If he gives the “green light”, administrative staff will schedule a date and offer advice on hotels.

Finally the client is filmed drinking the lethal solution of barbiturates in water to prove they took it themselves.

Those who cannot lift a glass press a button so a machine administers it.

Most people take between 30 minutes and an hour to die.

Brits who've died at Dignitas

MORE than 200 Brits have died at Dignitas in the past decade.

One of the most controversial deaths was in 2006 when terminally ill Craig Ewert, 59, was filmed dying at the clinic for a television documentary.

The programme, which sparked fury from anti-euthanasia groups, was the first time a suicide had been shown on British TV.

Retired university professor Craig had motor neurone disease.

In February 2009, millionaire husband and wife Peter Duff, 80, and Penelope, 70, who both had terminal cancer, were the first British couple to die together at the centre.

Top orchestral conductor Sir Edward Downes, 85, and his 74-year-old wife Joan died at the clinic five months later.

Lady Downes had terminal cancer while her husband was nearly blind and becoming increasingly deaf.

Daniel James, 23, who was paralysed in a rugby accident, was the youngest Briton to die at the clinic.

His parents Julie and Mark James, of Sinton Green, Worcester, took him to there in 2010.

They said the ex-England under-16 rugby player had repeatedly said he wanted to die.

The CPS said it was not in the public interest to prosecute his parents.

No one who has helped any of the Brits to die at Dignitas has been prosecuted.

Suicide is not a crime but it is illegal to encourage or assist suicide while in England or Wales, regardless of where the suicide takes place.

The majority of clients at Dignitas take between 30 minutes and one hour to die.

Voice of the Mirror: Dignity is a right too

Assisted suicide is a deeply emotional and ethical issue which understandably creates strong feelings.

Our report on an 83-year-old with dementia who ended his life at the Swiss Dignitas clinic adds another dimension to the debate.

This paper believes both sides of the argument should be heard and respected.

Some campaigners will fear this case could lead to a relaxation of the rules and place pressure on the vulnerable who feel they are a burden on their family and loved ones.

Others will argue the laws should be changed so those who are dying and feel they have no quality of life do not have to travel to Switzerland to end their life in dignity.

Nor will they think it is right that those who assist in such deaths, out of compassion, should be liable to prosecution.

Lord Falconer, a former lord chancellor, is seeking to change the law to make assisted dying legal for the terminally ill.

Any such legislation must be sensitively crafted and we should consider carefully before extending such rights to people with long-term conditions such as dementia.

There is much debate to be had but it would be wrong to ignore the wishes of those who, in very rare cases, want to kill themselves.

This included all mentally ill, and all enemies of rulers or ruling elites. Public torture is an extremely effective political tool. Not for the extraction of confessions, though. But torture one poor victim cruelly to death, and every onlooker will get the message: do not challenge authority!

Back when I was in school, I performed a scientific experiment in which I poured a liquid of one color into a beaker that contained liquid of a different color. At first the liquid in the beaker was diluted, but as I kept pouring, the poured liquid eventually overtook the liquid in the beaker, creating an entirely new substance.

That's what is happening in Europe as thousands of migrants flee their home countries, seeking refuge in the European Union. Germany, alone, is expected to have received 800,000 migrants by the end of the year, four times last year's number.

German Chancellor Angela Merkel has suspended the EU's Dublin Regulation for Syrian migrants. Under the rule, writes The Telegraph, "...migrants can only apply for asylum in the first EU member state they enter, and face deportation if they try to apply in another. But Germany ... has now ordered its officers to process applications from Syrians even if they have made their way through other EU countries."

Critics of the Dublin Regulation have called it expensive and ineffective. Many Syrian migrants reach Greece first, but decline to seek asylum there, preferring to push on toward Hungary, which is considered the gateway to Europe. Merkel has said she might reinstate border checks, which was a regular practice before the EU was created. She should.

That most of these migrants come from Muslim countries raises several important questions. The first is how many actual or potential terrorists are among them? Second, why would Muslims, many of whom believe the West is decadent and anti-God, want to come toEurope? Third, why haven't these migrants sought refuge in other Muslim countries, which one might think would be their first priority?

No nation, no continent, can survive uncontrolled immigration, especially when it involves people whose language, religion, culture and worldview differ -- in some cases radically -- from the countries to which they are migrating. Uncontrolled migration in Europe and illegal immigration in the United States spell an eventual death knell for both countries, which is, no doubt, the intent of ISIS, which is reportedly backing this flood of humanity. Will those flooding Europe eventually embrace European values, or when their numbers grow to the point where they form a significant percentage of the population, will the migrants demand that their values and religion dominate?

In the U.S., cries of "racism" in the immigration debate have replaced sound reason. But this isn't about discriminating against people of a different language or color; it is about preserving what we have, not only for those of us who have contributed to making America what it is, but also for immigrants who would not only like to partake of our success, but contribute to it. If nations want to preserve the lifestyles and culture which they and their forebears have worked and fought to create and sustain for themselves, their posterity and for immigrants, borders must be controlled and assimilation must be a top priority for those who are allowed to enter. Otherwise, nations become disunited with competing subsets jostling for power, influence and benefits.

Those who support "open borders" have an obligation to tell us when enough is enough. Must we wait until the American "liquid," which contains the values, faith and prosperity from capitalism that built and sustained us through wars and depressions, is replaced by a different "liquid"? What happens when the "takers" outnumber the "makers"? If we wait for that day to arrive before we act, it will be too late.

Yes, give us your tired, your poor, your huddled masses yearning to breathe free -- but legally, in an orderly fashion and not en masse. And let's also learn what these migrants and immigrants likely mean by "freedom." If they mean Sharia law, that is not freedom for Europe, or for America.

If the EU and the U.S. fail to address this very real crisis, we and they are assisting in national suicide.

Numerous scientific studies have shown that properly concentrated tongkat ali extract (not the fakes and not the junk commonly sold as supplements) will raise testosterone levels. The rise of testosterone during puberty is what causes penis growth in boys [1][2][3]. Depressing testosterone synthesis through exogenous hormones in bodybuilders causes penile and testicular shrinkage [4][5] . All of this is well documented in the scientific literature, and even in college textbooks.

Tongkat ali has been shown to raise testosterone. It is alleged that it does so by stimulating the Leydig cells of the testicles to produce more testosterone. This is different from the action of testosterone products, which act like the body’s own testosterone and therefore causes the Leydig cells to shut down their production of the hormone. For this reason, all exogenous testosterone products, including anabolic steroids, cause testicular and penile shrinkage.

I do think that this is not sufficiently emphasized by anti-doping campaigns: athletes who dope with testosterone and anabolic steroids will cause their penises to become as small as their thumbs or little fingers, and unfortunately, if exogenous testosterone or anabolic steroids are taken for some time, the loss in size can be permanent. The effect of tongkat ali is the opposite. Because tongkat ali stimulates the body’s own testosterone production in the Leydig cells of the testicles, the testicles and surrounding tissue will grow in size. (During puberty, it’s a spur in Leydig cell activity, which causes a boy’s sexual organs to become the size of a man’s. Furthermore, testosterone over-production endocrine disorders during adolescence cause oversized male genitalia [6][7] .) While a large number of men may consider increased penile and testicular size a benefit in its own right, the mass gain also influences sexual function, especially erections and ejaculations. The erectile and ejaculatory support of tongkat ali is, so it appears, a direct consequence of then increased size of male accessory sexual structures.

Critical organs in ejaculations, as well as erection firmness, are the levator ani muscles of the pelvic floor. These muscles can be trained through Kegel exercises (youtube video see here)(an essential part of commercial penis enlargement programs), which, however, are very time-consuming for limited results. On the other hand, it has been shown in a scientific study that tongkat ali can cause a substantial increase in the size of these muscles in particular (an off-shot of the increased activity of testicular Leydig cells).

A scientific article published in the Archives of Pharmacological Research has now shown that tongkat ali can cause a size increase in male reproductive structures. I do want to emphasize two findings of the above study:

1. the statistic size increase of the levator ani muscles was “significant”.

2. the dosages used were 200, 400, or 800 mg of extract per kg of body weight; this means that for a man of 80 kg, the equivalent dosage would be 16, 32, or 64 gram of extract per day.

3. the tongkat ali extract was used for a full 12 weeks (3 months) without interruption.

Even for the lowest dosage used in the above-cited animal model (200 mg per day per kg of body weight, or 16 gram of extract per day for an adult man), a full bottle of most commercial tongkat ali extracts wouldn’t even provide enough for a single dosage.

Because tongkat ali isn’t exactly a cheap herbal (as it is harvested in remote jungles), under-dosing is very common for the producers and distributors of commercial tongkat ali products.

Traditional usage in Indonesia is about 50 gram of root, boiled for half an hour. This would be equivalent to 1 gram of 1: 50 extract.

The lowest dose used in the above study was 200 mg of extract per kg of body weight, which would mean 16 gram of extract per day for a 80-kg man. Assuming a 1:50 extract, this would translate into 800 gram of root in a single dosage.

A common product on US health food store shelves offers 50 mg of a 1:100 extract. Do your math. 1 capsule contains the active ingredients of just 5 gram of tongkat ali root. To repeat the above animal-model study, one would need 80 capsules per day.

Of course, because tongkat ali is expensive, some manufacturers who do not want to spend on genuine tongkat ali root stretch their products with cheap bootleg chemicals, mostly Pfizer’s Blue and Lilly's Beige. Pfizer’s Blue is the active ingredient in Pfizer’s Blue, and Lilly's Beige is the active ingredient in Lilly's Beige.

However, those who sell stretched alleged tongkat ali do not use Pfizer’s Blue or Lilly's Beige from proper pharmaceutical factories, but rather turn to cheap street Pfizer’s Blue and Lilly's Beige that has been produced in kitchen labs in China, and is smuggled to Malaysia where it ends up in alleged tongkat ali capsules.

The San Jose Mercury News reported on May. 11, 2004 that “researchers found that Super-X contained the key ingredient in Pfizer’s Blue. A similar herbal supplement called Stamina-Rx contained the active drug in Lilly's Beige, another erectile dysfunction drug…”

Stamina Rx is still sold over the web, even though “in 2003, the FDA warned Hi-Tech Pharmaceuticals, the Georgia distributor of Stamina-Rx to stop marketing the drug.”

In Malaysia were tongkat ali extract has become more than 20 times as expensive as Chinese bootleg Pfizer’s Blue, the New Straits Times has reported that the Ministry of Health has moved against local tongkat ali companies.

While I have mentioned above that for many commercial tongkat ali extracts, one would have to swallow all the capsules of a bottle to get a decent dosage of tongkat ali extract, this is actually not recommended because of the uncertainty about the presence of kitchen lab-manufactured pharmaceuticals, such as bootleg Pfizer’s Blue, and Lilly's Beige.

For a link between tongkat ali sales and the sales of the Stamina Rx supplement cited in the San Jose Mercury News article, see tongkat-ali-herb-testosterone.com. I obviously do not recommend to purchase from them. The reason is that in products that contain illegal Pfizer’s Blue or Lilly's Beige, the drug will likely not come from an official manufacturer but a garage lab in China. There may be all kinds of impurities that can have a very serious impact on a consumer’s health.

Please note that while the above-cited scientific study used 200 to 800 mg of tongkat ali extract in the animal model, the manufacturer-recommended human dosage for the 1:50 extract is 1 gram for average users and 2 gram for bodybuilders. I often take 3 gram, but I am a long-time user and I assume I have a good tolerance for the herbal.

If ever somebody should consider to provoke significant penis and testicular size increases with still higher dosages, a proper way would be to increase the dosage gradually, and not to jump to, let’s say, 10 gram without previously having tested 8, 6, 4, or 2 gram.

As with any effective medication, one can overdose on tongkat ali. While I have never heard of a human fatality, overdose experiments have been made with lab animals. In a study published in the Thai Journal of Phytopharmacy in 1998, it was established that the LD50 (the dose at which half of the lab animals died) for tongkat ali water-based extract was above 3 gram per kg of body weight. For a 80-kg man, this would be the equivalent of 240 gram (which is more than 100 times the manufacturer-recommended dosage for body builders, and 15 times the dosage at which an increased size of sexual accessory structures was noted in the above animal model). Not surprisingly, a prominent feature of the tongkat ali-poisoned animals was the heavily increased size of their sex organs.

It has to be mentioned, though, that while there is a lethal dose for tongkat ali extract, there are numerous health benefits at standard dosages.
